The ‘Tobramycin dosing calculator for children with cystic fibrosis aged
1-18 years’ is a dosing calculator for individualised
tobramycin dosing in children with cystic fibrosis.
It recommends the dose required to achieve a steady state AUC24 target of
95 mg/L·h based on an individual’s age, weight, height, and
serum creatinine level.
This calculator is based on a published population pharmacokinetic model (Harwood et al, Antimicrob Agents
Chemother 2025 0:e00278-25)
This calculator is intended for use in children aged
1 to 18 years with a weight of 7.5 to
82 kg and height of 0.7 to 1.85
metres.
It has not been validated in children:
- without cystic fibrosis, or
- those with abnormal renal function, or
- those receiving extracorporeal membrane oxygenation or renal replacement therapy
This calculator is intended to be used alongside routine therapeutic drug monitoring.
